Consent Form

If you are involved in leading groups or events at St George’s or St Martin’s then it would be helpful to be able to provide your contact details on the appropriate web page.  However, we will not do this without your permission.

To give permission to display your contact details, and to choose which details to display, then you may complete the online form below.  Alternatively, you may download and print out a hard copy, fill it in and hand to David Coleman or to the church offfice.
